Carruthers studied at Manchester Medical School from 1862-1865 and at Edinburgh University. He was house surgeon at MRI from 1870-1871 before being appointed medical officer of health for Halton and Weston District. Carruthers was later appointed medical officer and public vaccinator for Halton District, Runcorn Union.
