Chadwick was educated at Rochdale Grammar School and Owens College Manchester. After qualifying, Chadwick worked as a general practitioner in Milnrow, Lancashire. In 1900 he was appointed medical officer of health for the district, a position he held until his death on 25 January 1926. Chadwick was very active in local government, especially in areas of public health and welfare.
