After qualifying, Fox held junior positions at MRI. He served in the RAMC in Malaya and Singapore before returning to the University of Manchester in 1960 as assistant lecturer. He became lecturer in 1964 and reader in pathology in 1974. In 1974 Fox was visiting professor of pathology at the Trivandrum Medical College, Kerala, India. He was appointed professor of reproductive pathology at the University of Manchester in 1978. Fox became honorary consultant pathologist to United Manchester Hospitals in 1968. Fox published extensively on pathology of the placenta. When he retired, some time in the 1990s, Fox was made emeritus professor of the University. Fox was president of both the International Society of Gynaecological Pathology and the International Academy of Pathology, British Division.
