Maccall gained his medical education at Glasgow University and was house physician to Glasgow Royal Infirmary. He was surgeon on the SS Iowa before being appointed honorary physician to the Clinical Hospital for Women and Children, Manchester. Maccall had practices in Kersall and Cross Street and later in Southport.
