Miller was born in London on 30 September 1935, the son of a dental surgeon. He qualified as a dental surgeon in 1960 and held junior appointments in Bristol, Manchester and Dublin, but later undertook medical training in Dublin. After qualifying in medicine, Miller held house appointments at Leigh Infirmary and Chester City Hospital. He was psychiatric senior house officer at Rainhill Hospital Liverpool and Withington Hospital Manchester before being appointed registrar in psychiatry to MRI. In 1973, Miller was appointed psychiatric registrar to the alcoholism treatment unit at Springfield Hospital. He was senior psychiatric registrar at Alder Hey Children's Hospital and at Whittington Hospital, Preston, before being appointed consultant paediatric psychiatrist to Royal Oldham Hospital. Miller later became consultant psychiatrist to Chorley District Hospital. He was a fellow of MMS and affiliate member of the Royal College of Psychiatrists. Miller was recorder, curator and librarian if the Lancashire and Cheshire Branch of the BMA.
