MacKenzie was born in Aberdeenshire on 11 April 1878. He gained his medical education at the Universities of Aberdeen and Manchester. MacKenzie was senior resident to Oldham Infirmary before being appointed medical officer in charge of Huntroyde Auxiliary Military Hospital and surgeon to Queen Mary's Military Hospital, Whalley, Lancashire.
