Gokal was MRC clinical research fellow and honorary senior registrar at Radcliffe Infirmary before being appointed lecturer in medicine at Newcastle University. Gokal was later appointed consultant nephrologist at MRI. Gokal was president of the International Society for Peritoneal Dialysis from 1998 until 2001.
