Mackay held junior appointments at Glasgow Royal Infirmary and Western Infirmary Glasgow before moving to Manchester. He was resident medical officer at Hope Hospital Salford for eight years before being appointed medical superintendent in 1941 after Dr J. Dudgeon Giles was killed in an air-raid.
