Smith studied medicine at Manchester and held resident appointments at MRI and Manchester Royal Children's Hospital. He was clinical assistant to St John's Hospital of the Ear for four years before being appointed honorary surgeon. Smith was also honorary physician to the Manchester Hospital for the Throat and Chest. Smith was a fellow of the Royal Society of Medicine and a member of the Manchester Medical and Pathological Societies. He was later surgeon to the Manchester Ear Hospital and ENT surgeon to Rochdale and Bury Infirmaries.
