Smallman studied medicine at Owens College, winning a number of academic distinctions, and then held house appointments at Salford Royal Hospital. He was a lieutenant colonel in the RAMC and was assistant to the Director General of the Army Medical Services. Smallman later became medical officer to the Ministry of Health. He was a fellow of the Royal Society of Medicine.
