Smalley gained his medical education in Manchester and held junior appointments at the MRI, Derbyshire Royal Infirmary and finally at Royal Ear Hospital, Soho. During the first world war, Smalley gained the Military Cross. He was later appointed honorary surgeon in charge of the Aural Department at Salford Royal Hospital and honorary surgeon at Manchester Ear Hospital. He was also visiting aural surgeon to Grangethorpe Hospital. Smalley established in Manchester a high standard of technique in delicate otolaryngological operations. Smalley was a member of Manchester Medical Society, Manchester Pathological Society, and MRI Old Residents' Club, and was a fellow of the Royal Society of Medicine. He retired to North Devon, and died on 10 April 1966, aged 78.
