This collection comprises four books of completed midwives forms for sending for medical aid, completed by J. E. Upton whilst practicing as a midwife around the area of Wilford, Nottinghamshire (6 April 1960-16 February 1972), recording the name and address of the patient experiencing complications during childbirth, as well as the particulars of why medical aid was being sought, the date of confinement, the name of the doctor whose was called and how the request was sent.
