Address was given on 15th January 1648. First line of text reads: 'My L° & gentlemen having by ye weekly occurrence of ye proceedings'. Text ends: 'Hee will in his good time heare and answer this prayer of all true Englishmen, and among them of yo[u]r lordships humble servant H. Hammond'.
This seems to be a copy of one of the editions printed in London for Richard Royston in Ivie-Lane, 1649.
In the manuscript index at the end of the volume this item is listed as: 'Dr. Hen. Hammonds address for K. C 1. 1648. fo. 575'.
