Each form requests the following information: Full Name, Address, Date and Place of Birth, Present Appointments, Date of admission to the College, Details and dates of Appointments within the College, Personal interests outside Medicine, Surgery and Denti

In the Spring of 1988 Council decided that for historical purposes it would be interesting and useful to maintain in the College an archival record of some Fellows and Members of the College. It was considered too large a task to include all Fellows and Members and the initial exercise was restricted to Past Presidents and the then Members of Committees of College and Council, Members of the Lists of Examiners, Overseas Advisers and representatives on Trust Funds or other outside bodies.
