Of the name of the place from which it was sent only the initial L remains, and of the date 'decimo KL Novembris Pontificatus domini nstri domini Julii divina provide[ntia]'. There was a Bishop Julius of Florence 1158-1182, but the document appears to date from the 11th century or earlier.
